Online Registration-Request Limitations

• I understand that submitting this form does not yet complete my enrollment.
• I understand that I am not enrolled until my payment in-full by cash or check, money order, gift certificate, valid Groupon Code, Amazon / Living Social Certificate Number has been received, AND that it must be received within 4 business days, or on the first night of class (whichever is soonest) in order to guarantee my pricing and space in class.
• I understand that in some special cases I may be allowed to place a "credit card hold" to guarantee pricing and available space in class, and that payment in-full by one of the above-listed methods is still due on or before the first night of class.

Non-Refundability and Transfer Policies

• I understand that cash, check, money order and credit-card hold registration fees are non-refundable and are only transferable to a future session if I provide more than 48-hours notice prior to the first night of class.
• I understand that Groupon, Amazon Local, and Living Social redemption codes, once redeemed, are non-refundable and non-transferable.

Newcomer Discount: Is this your first group class at the Ballroom? Save $10 off your total registration fee when you register by noon on the first day of class. (Newcomer Discount cannot be combined with Early Registration Discount.)

Pay By Mail ~ I am registering MORE than 4 business days before the start of class, and will send payment by US Mail.

• I will make my check or money order payable to "John Ross", attach it to a copy of my printed confirmation page, and mail it to:

• I understand my payment MUST be received within 4 business days for my early registration discount (if applicable) to be honored, and for my space in class guaranteed.
• I understand that if my payment is not received in time, that I must pay the full at-the-door registration price by cash or check in order to attend class.
• I understand that once paid, my registration fees is non-refundable and only transferable to a future session if I provide more than 48-hours notice prior to the first night of class.

Use Gift Certificate ~ I am registering more than 4 business days prior to the start of class and am redeeming a Gift Certificate.

• I understand that I am not enrolled until my gift certificate has been validated.
• I understand that I must bring the certificate to the first night of class and submit it for redemption.
• I understand that if I do not bring my gift certificate to the first night of class that I must instead provide payment by cash or check in order to attend class.

Redeem Special Offer ~ I am registering more than 4 business days prior to the start of class and wish to redeem a pre-paid online special offer (Groupon, Amazon Local, Living Social, etc).

• I have included my redemption code in the "Notes" field above.
• I understand that Groupon, Amazon Local, and Living Social redemption codes, once redeemed, are non-refundable and non-transferrable.
• I understand that if I do not provide a valid, unused, redemption code, that I am not enrolled in the class, and must pay the full-at-the-door price unless other arrangements have been made in advance.

Pay At Door ~ I am registering AFTER 12:00pm on the first day of class, and understand I must pay the full at-the-door price.

• I will bring a printed copy of this registration request to speed-up the at-the-door registration process, or I will arrive 15 minutes early to fill-out the paperwork required to become enrolled in class.
• I will also bring my payment for the full at-the-door registration fee by cash, check or money order (payable to "John Ross"), or will bring my gift certificate, or printed special-offer redemption form for my pre-paid Groupon, Amazon Local, Living Social or other online special offer.

I have read, understand, and agree to the Shoe Policy, Group Class Policy, Missed Class Policy, and Payment / Transfer Policy.

I understand and agree that once received my payment is non-refundable, but when certain conditions are met, it might be transferable within 48-hours prior to the first night of class.

I understand that if I miss two classes in a row I must complete a make-up lesson prior to returning to the group class, and that this is a 25-minute private lesson with a cost of $33 to $40 per person or couple. (Cost varies by instructor and payment method. Please see our price list for additional information.)
